Brent,

Don't know what they're looking for. The 3 forms that you list are the only 3 that the examining doctor has to complete.

She does need to take with her 3 'passport-style' photos, and probably some ID such as her Thai ID card and Tabian Baahn - the examining physician does need to confirm that he's examining the correct person (so that someone who may be medically inadmissable to te USA does not hire a ringer to take the exam for them).

The "form with her passport photo attached" could conceivably be her DS-156.

Intersting that there are only 2 approved examination facilities in Bangkok now, there were 3.

Interesting also that the consulate web site identifies the medical examination instruction form as "BKK-MEI" - the DoS designation for the US embassy and consulate in Thailand is BNK. BKK is the airline industry's designation for Don Muang airport.

Hello all,

Wi went to get her medical today. She was turned away for only having the three forms we downloaded from the Bangkok Embassy Site. She was told to return when she has all six forms that are provided for her in packet 3.

We still have not recieved packet 3 from the Embassy. The three forms she has is DS3024, DS3025 and DS3026 along with BKK-MEI (Instructions)

What 3 forms is she missing?

If it makes you feel any better, the email for the IV unit at the embass is visasBKK@state.gov,lol.

C will call the hospital today to confirm the process here in Chiang Mai but we're under the impression that all that is needed are the forms from the packet 3 off the internet along with the pictures and maybe the NOA2 (?). I will post the results after she calls them today (thailand time).

12/15/2005 - Sent I-129F to Vermont Service Center

12/17/2005 - VSC receives I-129F via express courier (day 1)

12/23/2005 - VSC officially sends out NOA1 stating receipt

01/05/2006 - Called the National Service Center and got NOA1 receipt number.

01/10/2006 - NOA2 received via USCIS website and email. VSC is blazingly fast! (day 22!)

01/20/2006 - Receive NOA2 via snail mail in Thailand

01/26/2006 - Bangkok Embassy confirms our case is in their system (day 38)

01/30/2006 - Dropped off checklist and doc's with embassy

Can someone please comment here on what is needed for the medical? I have read some posters here who were able to get the medical exam without any packet 3 sent from the embassy. All we have is the packet 3 from the website. Does anyone know what this other form he's talking about is?

Could it be a new process :o ? When my wife did hers (back in Oct 2004), she only had the 3 DS forms (from packet 3) , 3 passport type photos , her Thai ID and house registration document (tabian baan) . Wi's report is the first I've heard about 6 forms needed for a medical at Bumrungrad.

Timeline

When Dao first went for her medical back in Oct 05, they would not accept the forms printed off the website. Fortunatly when she arrived home that day her "Packet 3" had been delivered. She went back a couple of days later with the forms from the packet and had no problems obtaining her medical at that time. I spoke with her last night and unfortunatly she does not remember what the difference between the web based and "Packet 3" forms was.

I have to tell you this is really confusing. My fiance is closing her business and flying to BKK today to take care of the medical, post office fee and the police report. I feel like i may have given her bad advice and lead her down the wrong path IF she is denied her physical because of the forms that we printed off the web site. It would be a terrible waste.

This could all be avoided if the Embassy would let her pick up packet 3....will they still give it out on request?

Su printed all the forms off the web site and everything passed through fine with no problems. This could be like the lady at window number 5. Sometimes you get the nice one and sometimes you get the mean one.

Tik will be going for the medical the first of March.. She has the 3 forms all of the forms say page 1 of 2 but only 1 has 2 pages. :help: We need some conformation on what we need soon so someone who finds out first please post asap. Also cost was 2400 baht for medical without shots and 5000 baht with shots.

I was going to say the same thing Tiksman said, which is I wonder if it is because the three forms in Packet 3 from the Embassy have 2 pages each (the second page being the instruction page), and the ones you print off the website don't?! :o

That would be a pretty cheesy reason not to do the medical, but who said anything in this process makes sense? :no:

Malydon, please let us know what happens with your fiancee. Maybe it is a case of talking to the "right person" at the hospital to get it done. Meanwhile, I'm going to search and see if it is possible to find the complete forms elsewhere.

After some searching, I found complete copies of all three medical forms, plus the DS-2053 form the other forms say they are to be used with. They are much cleaner than the forms on the "BKK" website! ;)

Anyway, here is the link, and I guess we shouldn't be surprised which Embassy it is:
