How to upgrade F2A to IR2

My wife should receive her naturalization certification on March 7. Currently, she has two F2A cases which were DQ'd last summer. I have heard conflicting information on how to upgrade them to IR2 cases. 

 

Does anybody know the exact steps?

Furthermore, during her naturalization interview my wife legally changed her name. The only difference is that she added a middle name. Her first and last names remain unchanged. Does this matter at all?

Did it three days ago. Do I need to upload the certificate of naturalization on CEAC as well or just wait for a response? Public inquiry turn-around time is around six weeks.
