What Form is Needed for Child Born..??

Timeline

Since you were a US citizen when your child was born and you have lived in the US for at least 5 years before the child's birth, then your child is a US citizen.

Google CRBA and your local US Embassy.  You will need to file a CRBA for your child and obtain a US passport for your child.

Do I filed this with the US Embassy oversea, or through the National Visa?

He doesn't need a visa since he's a US citizen by birth but you have to do CBRA with the embassy and then you can get his passport.

Yes, that's the first step.  Note that CRBA stands for Consular Report of Birth Abroad.  You get that first then the US passport for the child.  Depending on circumstances be prepared for DNA paternity testing.
