My apologies, I wasn't specific enough. I'm referring to form I-130 and the required documents to prove our marriage is legitimate. One of the documents they listed as evidence was a bona fide marriage affidavit. I'm asking if this affidavit needs to be notarized or not.

The answer is yes, but my advice is not to bother with it.  Concentrate on more meaningful evidence like evidence of time spent together in person.  Note that the list you are reading appears AFTER the words, "In addition to the required....".  Such affidavits are essentially useless.  They are the last resort of the desperate folks who have no OTHER evidence.

So was your marriage done by a notary?

that u should have stated

Religious weddings in Colombia are handled by individual religious officials and involve similar documentary procedures, along with additional religious requirements. After a religious wedding, the marriage must be registered at a notary’s office

So,  yes ,  your marriage certificate will have a notary signature

The OP is not asking about his marriage certificate.  The question is about affidavits from third parties used as evidence of bona fide marriage.

The problem is that I don't have a lot supporting evidence. We never lived together, she's never been to the US, We have no children together, my parents have never met her in person, we don't have a joint bank account or own property together. So I'm thinking it's better to err on the side of caution.

Yes, but affidavits from third parties are useless or worse.  How much time have you spent together in person over how many visits?  That's the most important evidence.  You're looking at a list of examples.  The last thing on the list is effectively "other".  Focus on "other" in the form of evidence of time spent together in person.
