Confused which name to use for AOS

I chose to take my spouse's last name as my new surname upon marriage, and this was reflected in our marriage license, but not in our marriage certificate. Our marriage license only asked for a new surname, nothing about my middle name. This is conflicting with the information I saw here https://nycourts.gov/Courthelp/NameChange/marriageDivorce.shtml, stating:

 

         You can choose to change your middle name and last name when you marry by putting the new name on the marriage license application

 

We live in NY, and upon confirmation with the city clerk, only my surname was changed.

My question is -  can I just fill up the AOS forms with the name I want (First, Middle (maiden), New Surname) so that my records (and everything else) will start showing my new name?

I hope you got the answer you wanted in time.

What made sense to me with this confusion is that my Maiden Name and my Married Name were both stated in the AOS. I used the Married Name already as my Primary Name since the Marriage Certificate legalizes that. and any old names I have used before such as the Maiden name were/was declared as Other Names used.

So what is reflect on the Marriage certificate that you have filled up while applying should be the same thing that should be used on your AOS.

Thanks. To be safe, I just changed my surname, but retained my middle name. Our marriage certificate (not license) reflects my entire maiden name, and did not show the option that I opted for surname change (even if I did, in the license application). Will update this thread.
