2 questions on I-751 form

We have two questions about form I-751


For the I-751 form for place of marriage should we put the city and state?

 

The second question is if the spouse of the petitioner completes the form does the petitioner check the box that says I have requested the services of and consented to   (spouses name) prepare this petition for me on page 6 than the spouse fills out the preparer section as well?

 In an i751 case the green card holder is the petitioner and the beneficiary, the US based spouse is only the spouse and should only sign the spouse section of the i751 if it is a joint case.

K1 Visa Arrived USA July 2017

Married August 2017

AOS Approved July 2018

 

Filed for i751 joint application May 2020

Fingerprints reused October 2020, and February 2021 and June 2021 (Yes 3 fingerprint notices)

Case move to National Benefits Center December 2020 for quicker processing from California Service Center

Oct 2021 out of processing time inquiry made, response May 5th 2022 that our i751 case will be addressed at our n400 interview

Combo interview May 16th 2022, in Sacramento

Approved June 08, 2022

 

Filed for Naturalization May 2021

Fingerprints reused May 2021

Combo interview May 16th 2022, in Sacramento, 

Approved June 08, 2022

Oath Ceremony completed June 29th 2022

The place of marriage is just a blank space and instructions don’t say anything. Should we put the name of the church or the city and the state?

City and state

 

I signed my name under “was this prepared by someone else” and put “prepared by spouse” under everything we ever filed.

 

The beneficiary signs everything as well if he/she is the actual applicant.

No. "Preparer" is if you had somebody else preparing the form for you, eg. a paralegal. You either put N/A or leave it blank if you prepared it yourself.

February 4th, 2023: Biometric appointment

June 15th, 2023: Case actively being reviewed

July 11th, 2023: Interview scheduled.

August 30th, 2023: Interview!

August 31st, 2023: Oath ceremony scheduled.

Sept 19th, 2023: Officially a US citizen!

The I-751 form we filled out has an expiration date of 12/31/2019 in the upper right corner but it is the one on the website. Is this the correct one?
