I-134 Affidavit of Support form show the Exp. 04-30-07, I am pretty sure this form is being used but It can be a little confusing with dates like that written on the form. Plus this form need to be notarized when returning it to NVC.....

Don't worry about that date, but I don't understand why you'd be sending the form to the National Visa Center.

Edition Date : 2/28/07. Versions 12/04/06, 7/14/06 and 6/17/06 are also acceptable.

Besides it is the consulate that decided the version that they accept, and most consulates will accept versions much older that are what are on USCIS site.

Also it is sent to the benificiary in the foreign country for visa interview, not to NVC, and yes, it needs to be notarized by the petitioner before sending it to the benificiary.
