My wife interviewed for her green card in November, was quickly accepted and paid her fee prior to moving to the US in December. She received her visa in her passport, and quickly received her SSN. We are still awaiting her official green card. Today, I went back onto our portal at the NVC and noticed a bunch of documents were rejected. I'm not sure how it was rejected, or if we need to reupload everything even though it was accepted prior to her interview and she was given her visa/SSN? Her visa expires in April and I want to make sure we don't have any problems. Anybody run into something like this before?

First, your wife interviewed for an immigrant visa, not a green card.  The visa itself is for one entry, so since she has already used it, the expiration date is meaningless.  The combination of the visa and the entry stamp next to it, act as a temporary green card for a year from the entry date.  Actual green card issue and mailing has been taking a few months.  Nothing to do but be patient, since it's been only a month to two months since entry.

No comment on what you see in "the portal".  An issued visa followed by lawful entry using the visa, along with the new immigrant fee payment, should be all you need.
