Driver License Form (mononym name)

So, i am about to fill in my driver license form. There is a question regarding my mom maiden name. I am sure in future there will be these questions regarding this.

Problem is, my mom name is mononym. Our culture also doesn't have rules about last/family name or adopting husband family name.

I am currently in process for my GC. With this whole process, i fill out any form that requesting my mom maiden name with N/A and put her name as first name without middle and last name. Should i wrote the "one single word name" on the "mother maiden name?

My husband cant answer this question too....

There is no part where its breaks like in the USCIS form, its just; mother maiden name:

If its breaks into 3 part (first, middle, last name) as all of USCIS forms i can make it all the same so it will be consistent. Also mother maiden name should be my mom family name before she got married? (or am i mistaken on this?)

What form are you talking about?
