I recently entered US on K1 visa, and am preparing for my AOS while waiting for our wedding.

In my home country, I had the physical exam and my first vaccine(MMR&Varicella) shot together, I got a DS-3025 after it. Then I had my second vaccine shot, but the hospital didn't give me an updated DS-3025.

Now I have:

1) DS-3025 with only one shot of MMR&Varicella record
2) International Certificate of Vaccination with two shots of MMR&Varicella and one shot of Influenza.

3) A translated certificate of 2 shots of covid vaccine.

 

My question is:

Do I need to see a civil surgeon to complete my I-693? Or I just need to attach the photo copies of my certificates?

Check the bottom section of your DS-3025 -- does it say that you completed vaccination requirements?  The appropriate box should have been marked by the panel physician.  If so, no need for an I-693.
