The ceremony was just a ring exchange at home/we weren't dressed up or anything at all

my fiance at the time was also asked about identity of joint sponsor, maybe that was it?

im just nervous about cr-1 now given this

Is the joint sponsor someone that the foreign fiance does not know?

He knew the joint sponsor but she was not a blood relative (she was a friend of my mothers).

Well yes, they may question why an acquaintance would do such a thing.

Maybe look for a different joint sponsor, or look for work that has an adequate salary to sponsor an immigrant?

Probably something else if they did not like the joint sponsor probably asked for a better one

He thought the interview had gone well and he werent told at the interview anything nor were we asked for more evidence. They kept his passport and a month or two later we found out he was denied

How did you "find out"?  Please provide details.  For Pakistan and some other countries, all males go through additional, extensive security background checks.  Perhaps there's something in that background check that caused the denial. If so, it's still there.

beneficiary does not have any arrests or anything nor did he attempt

to

go to US by any other visa

What did the letter say exactly?

That's your interpretation.  Please actually quote the letter.

I unfortunately dont have a copy of the letter but i explicitly recall that it did not mention any reason for denial. He also wasn't given 221g form at interview or asked for more info, neither was I

the letter mainly said the case is denied and will be sent back to uscis to expire
