Notary for CRBA (split)

Hi Everyone,

 

I won't be going for the Interview, and I'm really confused about Notarizing these documents. Most USPS have no idea about the form 5507.

 

Is it ok to notarize form 5507 (https://eforms.state.gov/Forms/ds5507.PDF) at any US notary, or it has to be done in front of a Passport Specialist/Acceptant Agent.

You should try to attend 

 

We strongly recommend, and retain the right to require, that both parents and the child be present for the CRBA interview. If you are applying for the child's first passport at the same time as the CRBA, please note that both parents and the child must be present.

Form DS-5507 may be signed in front of a notary or at a U.S. embassy or consulate. 

 

That is anyone in your state with a notary license and stamp/  try your bank as they usually have a notary on  staff

BTW  no charge for notary

I have a couple of questions hoping someone can shed some light on it.

1. Do I need to fill out DS5507 even though I (US citizen) father and my wife (foreigner)  will both attend the interview?

2. For 5 years presence-- would original W2's be enough for the past 5 years? I also have weekly paystubs for the past 5 years in question (I know, I am weird lol) and Tax Return transcripts that I printed online from IRS.gov as well as social security statement. Would these be enough or do I have to go full on crazy and print my bank statements for the past 5 years or so and car insurance documents?

No reason not to fill DS5507 have it notarized and have it with you.

W2, pay stubs as well as utility bills that show name and address for the past 5 years will work.

I had all my CRBA paperwork notarized at my bank and had no issues at all. In fact, I called the Embassy in Manila to ask and they said any state licensed notary would be good.

interesting about the notary part. I was told no need to notarize anything unless I, U.S citizen parent, was not present at the interview. I am taking original w2's and paystubs. Do I need to make copies for the consulate?

That makes sense as you could just sign them in front of the embassy officer.
