My wife entered the USA on 8 December 2021 on a K-1 Fiance Visa. We were married on 11 December 2021. Prior to departing her home country, she opted to complete the immunization requirements after arrival in the USA and marriage to me. Now it is time to complete the immunization document. My question is: What form do we use? Is it the DS 3025 Form or must her immunizations be documented on Form I-693?

You now have to find a USCIS authorized civil surgeon to complete her immunization record/medical. Most likely she’ll end up doing the medical exam all over again unless she finds a civil surgeon willing to do just the vaccination part.
