uk medical gp referral for ongoing condition

it says i need "Referral from general practitioner (GP) regarding any ongoing medical issues or treatments" for my medical exam i have an acute condition/illness and i went into my GP to ask for one and they said they have no idea what this is. how do I ask for it, what do they need to write ?

They (LONDON clinic) want a brief report or statement in medical terms from the doctor who knows you. Name of condition, when started, ongoing or cured, treatment, meds, prognosis…like handled with medication or cured/healed…whatever. It can be just a paragraph or so. They just want to hear it from your doctor and not your version of it.

 

This also applies to any surgeries you have ever had, or things like high blood pressure, depression/mental health issues…major issues past or present, not a sore throat or hangnail.
