Tourist Visa Application Question

I am filling out the tourist visa application for my wife, for renewal.  On all the name fields (applicant, traveling companions, etc) it has Surname and Given Names.

 

In the given names boxes, should I put first and middle names?

 

Her Philippine passports, it has Surname, Given Names and Middle Name.

 

On her expired US visa, it has Given Names as the First and Middle names that are on her passport.  E.g.

 

Passport:

Surname:  Smith

First:  Mary

Middle: Jane

 

Old U.S. Visa:

Surname:  Smith

Given Names:  Mary Jane

 

On U.S. passports like mine, they have given names as First Middle.

I remember puzzling over the exact same question when I filed for a tourist visa.

 

In those fields, I only entered my First Name and Last Name. But when I got my passport back with the US tourist visa, I saw that they printed my middle name on the visa anyway, exactly how you described your wife's old US tourist visa. 

 

In short, I don't think it's a big deal. They'll print your wife's name according to their internal naming convention.

I will put the first and middle name in the Given Names, as it does say Names with an s on the end.  This is basically the U.S. passport format.  As Adventine said, they will probably make the visa name to their standard, which is the U.S. passport method.

 

My wife is also doing a name change on the "interview waivered" tourist visa renewal, which is allowed.  Change from previous married name to new married name.
