I got it done through the mail for $10.

I know... wish I'd known about that doctor when I got mine done - now I suggest that one to everyone who's looking for a civil surgeon! :)

They were really good about it. They let me fax it to them and they let me send a check after I got it back in the mail. Only took 3 days. :)

That's fantastic... definitely beats trekking halfway across town/the state/the country only to wait forever in some crappy office, potentially have to explain that no, you DON'T need a full medical, and then get charged an arbitrary, made-up-on-the-spot fee for a couple of minutes of form-filling (my experience wasn't too bad, but I know a lot of people have a nightmare trying to get the vaccination supplement)... that Dr. Arnold rocks!! :thumbs:

2005 - We met

2006 - Filed I-129F

2007 - K-1 issued, moved to US, completed AOS (a busy year, immigration-wise)

2009 - Conditions lifted

2010 - Will be naturalising. Buh-bye, USCIS! smile.png

Thanks for that, Gary! I am going to call them later, was hoping they'd let me fax it. I would be willing to give them a credit card if they wanted the $10 right then. :D

Oddly enough they don't do plastic at all. Just check and M.O. .

Probably because they'd be paying a merchant fee to the credit card company, and they are providing the service for bargain basement price anyway.

Yep - Dr. Arnold's office was great. I called left a message, they called me back, I faxed her the DS-3025 and other immunization records from a hospital to cofirm that my husband's vaccination were complete, the office confirmed it. I mailed off the copy of the DS-3025 and hospital records, name of country that my husband was born in, his DOB and check for $10 written out to the DR. L.E. Arnold. In a about the week I got back a copy of the completed Supplemental Form to I-693 and the orginal was in a sealed envelope that I left sealed and sent off for AOS.

Mine was also handed over as sealed. Infact the doc's office instructed us not to open it.

They kept a copy for the same and did show me the contents of the I 693 A but did not allow me to keep a copy.

So i have enclosed it as sealed and mentioned that in the cover letter as instructed by one of our VJs above.

Hmmm I have a question about this vaccination suplement. They gave my SO a copy of a form from the medical that is called :"US Dept of State Vaccination Documentation Worksheet" to be used with DS-2053 and the form expires 05/31/07. It list all the shots and certifies he has all of the necessary stuff. It was signed by an Amsterdam doctor.

Do I need to get another form? I thought this one was good..... :unsure:

They gave my SO a copy of a form from the medical that is called :"US Dept of State Vaccination Documentation Worksheet" to be used with DS-2053 and the form expires 05/31/07. It list all the shots and certifies he has all of the necessary stuff. It was signed by an Amsterdam doctor.

Do I need to get another form? I thought this one was good..... :unsure:

I believe that is the form you take to a civil surgeon here in the US to get it transfered to a I-693.

:yes: Although the form the civil surgeon will transcribe it to is the I 693 A.

Yes, that is what I put in the mail just today for Dr. Arnold. I also included a photocopy of my fiance's vaccination record (from his GP in the UK), but that is probably overkill on my part.

Oh and a $10 check. :)

So did mine... but then, I think it was the same one you used! (Sadly, not the same price, though... I think they just went for the 'think of a number' technique when deciding how much to charge me...) ;)

I fought hard to get that fee, as when I called it was what they said. When I showed up he tried saying $40. What did you pay?

$40... couldn't be bothered arguing it, as I had what I needed and know from reading posts on here that others have had to pay much more!

I had mine today and paid $50. Maybe because we had to sit and wait for it as they work on it (talking about the pressure we gave them, ha). They insisted that I do all the entire medical all over again but I showed them the instruction from the GUIDES. They asked me where in the world I got that instruction and so I told them. They checked it themselves and finally, worked on my I-693A. Ha!

We paid $25 for the I-693A. In our case, the civil surgeon also insisted that we get another medical....yeah right. Finally, she was like "whateva" and filled out the supplement. The bottom line is that you have to TELL them what YOU want.
