I have lived at the same address my whole life, what do I put as the start "Date From" date.  My birthday, when I turned 18, any date >5 years ago?

For Beneficiary work/address history, Thai addresses are very long and don't fit well in the fields on the I-29F form.  Can I just attach a piece of paper and list the full address and date on it?  Same question for work history.  What would I fill in 14a in this case, "See Additional Information" or leave blank? 

 

Is it ok to use estimated dates if you do not know the exact date (only month/year known)?  Example: Jan1 2018 - May31st 2018.

Yes, just remember to "indicate the Page Number, Part Number, and Item Number to which your answer refers; and sign and date each sheet."
