2nd EAD Renewal

Hi everyone, 

I need to submit my second ead renewal application. For supporting documentation should I provide documentation for my current (and second) ead, my original (and first) ead or both? I plan on submitting the ead front and back and NOA’s associated with both. 
also, can this be mailed as normal or can I do this online in my uscis account?

I-765 for (c)(9) category can only be filed by normal mail (or FedEx, UPS, etc.) since it's not on the following list: https://www.uscis.gov/file-online/forms-available-to-file-online

I-765 | Application for Employment Authorization

Filing under one of these categories:

  • (c)(3)(A) – Pre-completion OPT;
  • (c)(3)(B) – Post-completion OPT;
  • (c)(3)(C) – 24-month extension for science, technology, engineering and mathematics (STEM) students; or
  • (a)(12) – Temporary Protected Status (You are submitting an initial Form I-821 or you have an approved Form I-821).

last last question (I promise), did the supporting documents change? I’m planning on submitting the following for both the ead and the advance parole in the one package:

- application form (I-765 and I-131)

- g-1145 form

- passport photos x 2

- passport bio page

- I-94

- I-485 NOA

- current ead photocopy (front and back) 

- copy of current ead year off portion 

- current ead NOA2 (approval notice)

One thing I just realised is the USCIS online account number question on the I-765 form (question 9). 
Should I include the account number I was given earlier this year for the first renewal (the receipt number was IOE and the NOA came with a Uscis account number) or just leave this blank?
