Hello! k1 Fiancé had the full medical exam, chest X-rays, vaccination series abroad. Afterwards, We were only given a sealed envelope and were told to NOT open it, give it to the consular officer at K1 exam. This is what we did. Never saw the contents of inside of the medical packet. But all tests were completed and vaccines were done, the K1 visa was approved without question.

Entered the US and gave another sealed packet to the immigration officer at the border. Again, never seeing any of these documents as they were all sealed.

 

Now married, fast forward and we are applying for Adjustment of Status. Mind you the medical exam was early august and we are now applying for AOS so about 3 months only have elapsed. I now hear about submitting a copy of our “DS3025 vaccination records” as a part of AOS. But we NEVER got anything aside from whatever was in that sealed packet.

have tried to contact the medical office and embassy but not having much luck. What do we do??? Surely someone has the original copies????

The original is in the sealed packet you have at POE. Including a copy of DS-3025 is not a requirement but it is strongly recommended so that the IO knows you did have a medical exam and they don’t mistakenly send you an RFE for a it thinking you didn’t have it yet. 
 

Your only option is to keep trying the medical office. However, they’re not required to send you anything. For example, medical office I went through specifically stated on their instructions they do not issue copies of DS-3025 after the interview and to make sure to ask for a copy on the day. If they don’t answer you, you don’t have any choice but to file AOS without a copy of it.

What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it.
