give your name exactly as it appears on your Arrival/Departure Record (Form-I-94)

and on the I-94 First asks

Family Name ---------------

First (Given)Name ---------

so should to put my family name first and then my name ?????

on the first line and the right side says.. File Number. A .. what doest it mean exactly is it mean My Alien Registration Number ???

It means that you put your name exactly how you put it on the I-94 when you entered the country. Family name is your last name and the other one is self-explanatory. So yes, put your last name first and then your first name.

A number is your alien registration number.
