Question on EAD card ? Do I use same form as before ? Are do they have one for the 3 rd time application ?

2 photos , copy of current work permit, Uscis recite letter . 
Is their any thing else ?

No. Both I-765 and I-131 are still free: https://www.uscis.gov/feecalculator
