Requirements before leaving PH

I've received my Visa a month ago and just completed my 2 doses of my vaccine. Is it mandatory to get the International certificate of vaccination or "yellow card"? ill be receiving my Vaccine Certificate from my local city health office by Friday and I've read online that I also need that yellow card to be able to fly out of the Philippines. I'm just stressing out because slots are being taken way too fast. Also is the orientation pre departure still a thing? so I can book an appointment

Hello same po tayo kasi gusto ko sana magpa schedule sa "yellow card" when i saw their website Dec pa may available. Same question if needed ba talaga siya kasi walang slot.

as of now po 2 sites lang meron sakin that have available slots sa October and November, the rest is January na ulit available, may I ask what location po Inyo? there was a post from last month sa embassy website saying you may now get your yellow card etc, then upon checking ulit it did not mention anything about yellow card unless you've taken your vaccine from outside ph that's why now its confusing to me

Cebu po, Central Visayas. Nag check ako Dec pa ang merong slot and 2nd check ko Nov pero nawala and back to Dec ulit. Opo e confusing, gusto ko sana makakuha kung needed talaga siya para smooth ang travel.

aside from that naman are we required still to go through orientation? my brother had that when he flew to the US years ago, now lang sa pandemic di ko alam kung kailangan pa ba yun

CFO is required.  It is all done online currently.

hi thank you so much for the info. Do you also know if ICV is needed for travel to U.S? a lot of slots for the appointment are being taken and won't get one til dec or next year

If it is an immigrant visa CFO is required.   Info on the CFO site

You should register for it as soon as possible.  There is no expiration date on the certificate.  Everything is done online and the CFO "course" is done over the phone.  As soon as you complete the CFO, they will email you a temporary certificate within a week that can be used at the airport.  We left on the temporary certificate in February this year and many others on this board have done the same.

my remaining question now is whether or not I need to get a yellow card from bureau of quarantine, is it needed when flying to the U.S? I'm just waiting on my vaccine certificate by Friday

 The Yellow Card from BOQ is not necessary to leave the Philippines or to enter the US.  Some are recommending the BOQ Yellow Cards as potentially useful as more and more destinations start having vaccine mandates.  Mainly because the LGU vax cards look kind of easy to forge and cannot be verified overseas whereas the BOQ Yellow Cards can be verified overseas.  I think Hong Kong may be an example where LGU vax cards may be rejected and BOQ Yellow Cards may be accepted.  For US or just for exiting the Philippines it is a non issue.

for my vax card they made a mistake with my 2nd name , instead of Annamarie it was made as Anna Marie, I told them it was a mistake but they said it was okay, will that cause problems when i go to the airport?

Don't worry about the vax card.  As of now US still isn't checking them at all, and if they start I am sure US would accept a LGU card with a minor misspelling.  You will need a negative COVID test within 3 days of departure.  For US a rapid antigen swab test is fastest, easiest and cheapest.  CFO is still a thing and would probably be the biggest pre departure hassle.
