I clearly remember throwing away our DS-3025 when we packed up our apartment in Mexico and moved to the USA. How screwed are we? We have my finace's medical vaccination booklet, listing every vactionation he's had since childhood, but of course it is in Spanish. Do you think a surgeon would be able to transfer that to the I-683A?

Has anyone else mistakenly thrown away or lost that form they give you at the medical? Help!

July 12 WE FINALLY GOT MARRIED!!

AOS/EAD/AP:

July 2007 sent in AOS paperwork

Dec 6 CARD PRODUCTION ORDERED!

ROC:

Sept 5th 2009 Applied for Removal of Conditions

Sept 14th NOA I-797C

Oct 7th Biometrics Appt

You got a copy of the DS3025? We didn't even get one...it went directly from the clinic to the agent at the Embassy and then to the agent at POE.

But a few people assured me that this info would be entered into the system and that I didn't need it (although now I'm nervous because I read mention of including it in the AOS paperwork).

But the surgeon should be able to just transfer the info from his vacicnation booklet. I had to do a lot of talking at the civil surgeon's because she didn't know that she could just do the supplemental form and not teh full medical.

I went to the civil surgeon with my worksheet, however she complained that she couldn't read my doctor's handwriting and ended up not using it. She used the original vaccination records I had instead to transfer the information to the supplement. See if you can find a Spanish speaking Civil Surgeon and you should be able to use the original documentation without a problem.

Monica, are you saying he got his vaccinations in CDJ and they gave you a copy of the completed form? I didn't think CDJ did that.

In any event, if you have the booklet, take it to a Civil Surgeon who speaks Spanish and ask them to transfer it to the I-693A. If that doesn't work, you may have to find a doctor who can give him the titre to show what he has been vaccinated for. Then that information can go to a CS to fill out the form.

Oh yeah, of course homeboy had no records of his shots as they disappeared from his mom's house, so he got a titre to check for MMR.

That reminds me, gotta call the clinic!

I swear getting the i-693 vaccination supplement has been the most stressed part of AOS... we are in month 2 of this mess. Every CS i call insists on the full medical. If i tell them i only need the supplement, they tell me to go to school to learn about immigration! Ya, i think it should be the other way around!

One dr said that he'll fill it out, but wont sign it unless i fork over $190 (full medical cost).

These civil surgeons should get a refresher course every year in order to hold their license. In the first section of their course should be "how not to rip off immigrants" :D

One dr said that he'll fill it out, but wont sign it unless i fork over $190 (full medical cost).

These civil surgeons should get a refresher course every year in order to hold their license. In the first section of their course should be "how not to rip off immigrants" :D

Are you call CSs in downtown Chicago or the suburbs?

Thanks kitkat! I actually found one near Devon & California that only charged $50. Yippee, I'm finally done with this, now I can send of the AOS package!
