You can add a letter if you want, but they are referring to the cover sheet they sent you with the barcode...AND....the entire letter they sent you.  Make a copy for yourself and send the original back.  If you do not follow their instructions, you'll be dealing with a denial instead of an RFE?

If you don't mind me asking, why do you have to send this form? The reason I'm asking is becuase I'm also waiting for my wifes interview but I've never heard of this particular form you are showing. Also can you please tell me what your DQ date is as well as the date you recieved interview letter so I can get a better idea of when my wife will recieve hers

This form is used when USCIS needs more information before approving the petition.  Most people never see it.  Carry on.

Yes this form is only for those who need to send extra information and documents to uscis before they make a decision. Maybe thats why she hasn’t received it cause you both don’t need it!
