I suggest you enter your old USCIS A-number so the adjudicating officer can more easily look up your immigration record.  But you will likely be assigned a new A-number later on in the K1 + AOS process.

The petition only requires proof of in-person meeting within 2 years prior to filing.  Bona fide relationship evidence is not required, but you may submit some, if you want.  Your I-94, lease agreement, records from your school/employment are all good evidence, supported by photos of the two of you together.
