DS-3025: missing MMR jab at time of exam, later administered privately at home. What now for AOS?

Hi all,

 

I'm a K-1 visa holder from the UK. I entered the US in July, got married to my US citizen spouse in August, and am currently preparing to submit my AOS packet.

 

At my K-1 medical exam in London (May 2021), I passed the overall medical but was missing a 2nd MMR jab from my vaccinations record. Annoyingly they refused to provide it to me that day as I had my 2nd COVID jab a few days earlier!

 

Instead my DS-3025 form was marked "K visa applicant electing not to be vaccinated at this examination", and I had the missing MMR jab administered before leaving the UK in July. I have a record of this vaccine being provided by the clinic, and I also had it transcribed to my official UK medical history records before leaving.

 

I'm now confused about what this means for my AOS submission. Can I include the DS-3025 and proof that I received the missing shot? Will this be accepted or lead to problems down the road?

 

Any advice greatly appreciated!

One continuing source of confusion for me is that I don't have a I-693 form, as that is with USCIS (right?). All I have is the incomplete DS-3025. So how can I therefore get a civil surgeon to transcribe it for me?

In this case, you must submit Parts 1. - 5., 7., and 10. of Form I-693."
