Intent to marry : Wet dated and signed? / Visa pic size

Hello, I hope you're all doing well. Can you please help me ? Im preparing the intent to marry and I have questions : 

 

- Do I type my name at the bottom, type the date, print then wet sign or Do I print it then write my name, date it (at the bottom) using a pen and wet sign it?  ( Please find attached below two examples)

 

 

 

- Also for the visa/passport type picture, the format I have is 5x5 cm (5.1x5.1 cm is requested) will it be accepted or do I need to take other pictures ? Thank you.  I know I'm overthinking. I just want to do it right. Your help will be appreciated. Thank you. Have a good day.

Either way is ok

you have identified yourself in opening sentence,  just a signature is fine

typing name and then signing under it is more formal
