Renewal of a lost work authorization

Hello! My work authorization was lost, but the deadlines for submitting a new one have already approached (the old one would have lasted 3 more months). I’m filling out a form for a new permit and have a question- do I need to indicate that the form is being submitted because of the loss of my old WA or because of renewal? Maybe someone was in a similar situation... To renew, I need to attach the old WA, but I have no card itself, I only have a stub from the USCIS with the permit number.
