Self-Employed Employment History

Before starting my current job at a major university, I worked for about 1.5 years as a freelance writer and Korean-English translator (circa 2018) and am not sure how to fill out the employment history. I moved around a lot during that time and had a wide range of clients. Do I simply put "Self-Employed" for Full Name of Employer? As for the address, I wasn't sure if I could put down my P.O. Box that I used for business at the time or if it has to be the place I mainly resided during that time... Any advice deeply appreciated!

Timeline

Self employed will do

put the home address that you wrote on tax returns 

IRS allows PO box if mail can not be delivered to your home so,  do which is on the 1040 returns

Thanks so much!! I double checked and my PO Box is on my returns for when I was self-employed, so I'll go with that.
