Please I’m about uploading my civil documents. My petitioner has a proof of change of name documents from uscis .

should I attach the proof of change of name to my birth certificate since my birth certificate bear the old petitioner name or I should upload the proof of change of name at other documents

Yes, you may upload it under the section for other civil documents.  Just clearly label it as the petitioner's change of name document.

Oh ok thanks but can I attach it to my birth certificate and make my birth certificate as page 1 and petitioner change of name as page 2 so that when the open my birth certificate they can see both together on one page

NVC says to scan and upload each document as a separate file, unless it requires a translation -- https://travel.state.gov/content/travel/en/us-visas/immigrate/the-immigrant-visa-process/step-1-submit-a-petition/step-2-begin-nvc-processing/scanning-and-uploading-tips.html/
