An x-ray is just a preliminary diagnostic tool for TB.  If an x-ray is, at all, suspicious, then they move to more precise diagnosis like the sputum test.  Below might be a reason sputum tests are more common there:

In summary, it took 13 months for approval of the CR-1.  It took 44 months for approval of the I-751.  It took 4 months for approval of the N-400.   It took 172 days from N-400 application to Oath Ceremony.   It took 6 weeks for Passport, then 7 additional weeks for return of wife's Naturalization Certificate..

we was not told anything for that and POE did not do anything

Since my wife's second medical didn't require sputum, I guess we were lucky. This line "class A or B  (TB)  req. Attn. of usphs at poe" is not in her visa.

I find SLEC to be a seedy looking place to me, I walk on the other side of the street when I pass it when I am staying in the Ermita area.  All the people going there and getting tested positive for TB has to make the entrance unsafe.  I used to see people lining up like at 5 am to get medical exams there.
