Hi There Paul & Lily,

Because your wife came over on a K1, she should have had a medical exam done in her home country prior to having the visa granted. If this was less than 1 year ago, then all your wife has to do is get a Civil Surgeon to fill out the I-693A form and seal it in an envelope. They will either use her vaccination form 3025 (I think that's what it's called) that was given to her when she had her medical done, or she can do what I did and simply take her personal vaccination records to the Civil Surgeon to transcribe onto the form.

You can locate a Civil Surgeon in your area by going to the USCIS website. The link is also on VJ many, many times, so maybe just search for "Civil Surgeon Finder" and it will come up in the search results.

Edit: here is the link to the finder on USCIS: https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV

You will see in the instructions in the I-485 that K1's with a medical exam that is less than 1 year old ONLY have to fill out the vaccination supplement (I-693A). When you call around to the Surgeon's offices to make the appointment, they will likely tell you that you need a whole new medical exam. DO NOT LISTEN TO THEM! Keep calling until you find a CS that will fill out the I-693A only.

A couple of more things:

Do not open the sealed envelope that the CS gives you- it must stay sealed when you submit it in your package. Ask the CS for a copy for your records.

Also- if you're unable to find a CS that will only do the I-693A, there is a CS in Austin, TX that will do it by mail for $10. Search "Austin Civil Surgeon" here on VJ and you will find his address and phone number. You can call them to find out what to do.

When was the exact date & year your Lily had her medical exam at St. Luke's? Check if it is still not a year old or close to expiring. I know of other Pinays who got RFEs 1-2 months before their medical actually expires.

If her medical record from St. Luke's is still current and valid, she may need only the vaccination supplement to I-693 or the so-called I-693A here in VJ. Check what vaccinations still need to be administered here in the US (you have options whichever is cheaper for you guys) through the vaccination documentation worksheet copy given to her at St. Luke's, and have a USCIS accredited civil surgeon transfer her files onto the I-693A, signed and sealed in an envelope.

Don't forget to ask for copies from the civil surgeon for you to keep. If her medical record from St. Luke's is still valid, don't go for a full physical. Shop around by calling and asking through your list of civil surgeons (the link is provided already by Jashley820) how much just for the needed vaccination and transfer of files.

Good luck!
