If the insurance company initially denies for lack of SSN ask for supervisor/higher up. Here is what the US government states: https://www.irs.gov/affordable-care-act/questions-and-answers-about-reporting-social-security-numbers-to-your-health-insurance-company

A5: No, it is not necessary for you to contact the IRS if you do not have an SSN or a TIN. You should advise your coverage provider that you do not have an SSN or TIN and give them the date of birth for each covered individual.

Contact SSA is you don't receive it within three weeks of US entry. https://www.ssa.gov/ssnvisa/Handout_11_1.html

IMPORTANT: Call us if you do not receive your Social Security card after 3 weeks or if you change your mailing address after arrival in the United States. If you call us, please have this letter with you. It will help us answer your questions.

Yes.  Legally insurers cannot require your SSN if you don’t have one.  
 

Practically it appears to be a toothless law.  Thus many if not most insurers violate the law.  One alleged work around is to use 000-00-0000 as the SSN.  
 

Note that once you get an SSN you are required to share it with the insurer.
