how much do you pay for a test?

I can probably get my insurance to cover the telehealth but for the cost of buying a test, there's not much difference.

But I should also look into if they'd cover me buying a test directly.

If you’re going to have your insurance cover the cost of the test or tele appointment, tell them that you were exposed to a person who was positive. Many insurances, but not all, will not cover test for travel related purposes.  Check with your provider.

The United States is now a country obsessed with the worship of its own ignorance.  Americans are proud of not knowing things.  They have reached a point where ignorance, is an actual virtue.  To reject the advice of experts is to assert autonomy, a way for Americans to insulate their increasingly fragile egos from ever being told they're wrong about anything.  It is a new Declaration of Independence: no longer do we hold these truths to be self-evident, we hold all truths to be self-evident, even the ones that arent true.  All things are knowable and every opinion on any subject is as good as any other.  The fundamental knowledge of the average American is now so low that it has crashed through the floor of "uninformed", passed "misinformed", on the way down, and now plummeting to "aggressively wrong."

Which airlines is this that has a specific requirement?

I have been flying American Airlines from Cancun Mexico to Dallas,  if you do self check at the kiosk they don't check anything you just give them the form you fill out saying you had the test and it was negative.

If you check in luggage they want to see you test results which is given to you my email, There is no way they know those are legit or not.

It must depend on where you are coming from.  I know that if you are traveling from Pakistan to the US, most of the airlines (Qatar, Etihad) require a negative PCR test taken a certain number of hours (not sure what the number is) before boarding.

Obligatory disclaimer:  Not a lawyer.  Posts are written based on my own research and based on whatever information is provided.  Consult an immigration attorney regarding your specific case.

That sounds like a specific policy for an airline or maybe Pakistan.  I have only been going to Cancun Mexico and fly a US Carrier called American Airlines, they are following CDC requirements which is a negative PCR or Antigen Test within 3 days of flying into the USA for almost anyone, I think 2 and under are exempt, but not sure I have no kids.

There is no forum or official paperwork for the test giver to fill out.

The world doesn't revolve around one person's Cancun trips experience whether it's how airlines check the covid result or if masks are required at airports or if sponsors are required for visitor visas or student visas. You can't apply one isolated case to everywhere.

One eg: My FIL just flew to US on his visitor visa, he had to show his original test report at the airport to be able to depart his country. Different countries, different airlines, different embassies, different policies.

If I am in Cancun airport and flying say for example Jet BLue and they start saying you need this and this which isn't correct, I tell them to pack sand and go to the American Airlines and buy a ticket and fly with them, or Southwest.

What kind of world do you live in where you have to fly a certain carrier?

That's a different scenario, in your original post it sounds like you have a ticket with one airline and can just switch it for another, it sounds like you're going and buying another ticket.

Most people don't have the disposable income and buy their tickets ahead of time.

Even refundable tickets costs more.

I am not most people, I have the disposable income and credit cards and cash on me at all times.

I would assume everyone knows you have to buy ticket to fly on a plane.

Do you have any trips you have made out of the USA in the last few months?  I would love to hear what was required for you to show or do to get back into the USA and what country you flew from to enter the USA.

Your disposable income and credit cards are useless references for anyone in this forum looking for practical advice.

Additionally, I don't see the point in making a condescending comment about people knowing how to buy a plane ticket. That adds nothing to OP's question, which was about accepted COVID-19 tests.

OP, everything is changing fast these days so if you are unable to get help through the airline you're flying with, you might want to get familiar with the requirements and print the order for reference. here: https://www.cdc.gov/quarantine/fr-proof-negative-test.html

The PCR test requirements on some of the the middle-eastern airlines are requirement of the countries they transit.  And you are correct, airlines and transit countries may well have requirements that are much more strict than the US and are not negatable.  If someone wants to change planes in Doha,  they must meet the requirements for changing planes in Doha, not just the requirements of the US.
