Never received a copy of my DS-3025

I am preparing to apply for Adjustment of Status and have a question on a copy of DS-3025.

After reading some of the discussion boards related to this topic, I am hoping to get a more concrete answer to whether mailing in a DS-3025 is necessary.

 

I had all the required vaccinations at the time of the K-1 medical examination in my country and was told by the physician that I got the needed shots. In other words, the physician did not do any vaccinations on me.  When I asked for the copy of DS-3025, the physician informed me that she cannot share nor provide me the copies of the medical documents as they needed to be delivered to the U.S. Embassy in a sealed envelope. The physician in my country never provided me a copy of DS-3025 nor the immigration officer at Point of Entry (POE) in the U.S whom I handed over my sealed envelope to. I suppose that the immigration officer at POE turned/sent over the original form with my other medical results to USCIS.

 

I would appreciate it if someone could share their experiences with submitting their Adjustment of Status without a copy of DS-3025 as a result of not receiving it from their physician and the immigration officer at POE.

Your panel physician is REQUIRED to furnish you with a PERSONAL copy of your DS3025 as per the CDC requirements TO BE an approved panel physician.

Contact your panel physician and show them the CDC website if they try to argue.

"The panel physician must review all vaccination records presented by the applicant and, if documentation appears valid, record the vaccination history and vaccines given during the medical exam on the US Department of State Vaccination Documentation Worksheet (DS-3025).  Both the panel physician and the applicant should be aware that the DS-3025 is the applicant’s vaccination record, and an extra copy must be provided to the applicant.  The applicant will need to provide a copy of the DS-3025 to healthcare providers, schools, and other institutions after US arrival."
