Dear Member - I'm in the process of filing i485 for my mother who is 72 yrs old and need some inputs on documents needed in lieu of her birth certificate.

 

My mother was born in a remote village and her birth was not recorded . USCIS reciprocity page states that for applicants born before 1970 , birth certificate is considered not available and we need to submit 2 separate affidavits from her relatives. However , my maternal aunt is the only elderly relative currently alive . So can we submit a self affidavit from mother as a second evidence ? If yes, can we get it notarized by an attorney here in US as my mother is currently in states?

 

"If the applicant has no living relatives that witnessed their birth, a self-attested affidavit detailing their knowledge of the facts of their birth may be accepted."

 

Any inputs is much appreciated 

 

Lastly , can someone share a sample self affidavit that I can reuse?
