Can US visas be issued without a passport?

I came across this article a few years ago.  In it, it is mentioned that since the beneficiary was unable to obtain a passport from his native country, he was issued the visa on a special piece of paper.  Out of curiosity, is this a one-off case, or do the INA/CBP rules have an obscure clause that allows this?

This is very interesting. I always thought passports were mandatory for visas to be issued. But it makes sense since he's a refugee from a failed state. How odd it must have been to present just the visa sticker to the airline and CBP!
