My FIL was denied visa first time for not passing medical but his medical result came back normal. He turned in the paperwork and got denied again. Is there a way to check the reason?

May 7, 2014 - I-130 sent to Chicago, IL Lockbox

May 12, 2014 - Case recieved at Chicago, IL.

May 13, 2014 - Case accepted. Received LIN#. Case forwarded to USCIS, Nebraska. i-797 hard copy received shortly after.

It does not say denied 

visas are either issued or denied

 

Refused seems to be the word they are writing instead of AP 

so,  not sure you are actually not getting the visa or it is being reviewed

 

and not sure how you know medical is ok/  we don't get told or get to see the results of the exam

The third sentence in the notice is key.  Refused does not mean denied.

Right. But is there away to find out the reason? Maybe he missed turning in something? Just that his case got updated today, I want to get ahead and prepare such document. My MIL is already here in the US for over a month now, just that he still stuck there.

Go back and review what your FIL was told by the consular officer at the interview.  As @JeanneAdil stated, the term “refused” appears to have replaced “Administrative Processing”, so there may not be anything missing, or documents necessary, but rather more background checks, or a review by a supervisor, etc.

A reason ?   there isn't one

A denied visa has a reason 

an  approved visa means all is good

one in AP (refused ) is just going thru the process for all documents and security checks / there  no reason like you want

I wish immigration would not use the word refused while people wait for a decision as it is now the most common question on here 

and sorry,,  but yes,  you must wait 

"hurry up (with documents and petitions ) and wait"  

and waiting can drive you crazy
