and then expect the embassy to say new medical (recent one) will be needed if they have are inclined to approve the visa (it still is not guaranteed as other things change in a year so stay out of trouble and stay healthy)

They can be obtained in the UAE if you are out of the country. I did exactly that. But it is difficult - I had a friend who speaks much better Arabic than me do it. The US waives the requirement in most cases because of the difficulty obtaining them.
