Regarding Dutch certificate of good conduct (VOG application)

Hi all,

 

I need to apply for the certificate of good conduct from Dutch justis to apply for VISA to USA, but I find the supporting letter on the US state website (https://travel.state.gov/content/travel/en/us-visas/Visa-Reciprocity-and-Civil-Documents-by-Country/Netherlands.html#ExternalPopup) is broken currently: http://photos.state.gov/libraries/netherlands/19452/public/IVNIVVOGAPPSUPP.pdf, may I ask if anybody know what is the corrent link for this supporting letter ? Or does anybody have a copy of this PDF file? If so, could you please send me a copy to my email:  I will appreciate very much!

Click on a highlighted date in the calender (e.g. March 30th 2021) and you'll see the form. Or send me a PM and I can e-mail the form to you!
