Vaccinations can be given by anyone qualified.  Bring your records to your visa medical so thr ds3025 can be completed properly.   Your vaccination history appears complete so stop worrying about the i693.

No.  Reread the instructions carefully.   It can only be used if it was in the medical that you handed over at the POE.   Medical information REQUIRES  chain of custody.   A photocopy of your personal record included with the i485 fails that requirement.

No one is saying that a photocopy of your personal record is valid. You need either the document handed to you by the doctor who did your K1 interview, or you need to do the medical over again and use the vaccination record you get from the approved doctor in the US. You need to send something in.

Then I guess I better send the greencard back because we didn't send something.

 

The ONLY time you need to send something is if your ds3025 is NOT marked complete, otherwise they are satisfied with the one in the packet from the consulate.

Not so. You do not have to send one or the other at the time of filing. From the pinned topic I posted a link to earlier in this thread>

What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly.

 

If someone gets an RFE before or at the interview asking for the I-693, then you will need to comply. No need to rush off to a civil surgeon for another exam before filing just because you don't have the DS-3025 copy in hand.

Also, I hate the doctor that did my examination back in Belgium.

The one in Antwerp. He also has no way to contact him, other than phone, so it's a little hard to get in touch with him getting sent the records.

Thank you so much for the reply! This is the doctor I go next week, hopefully everything will be ok 😃😩
