probs after the TD shots needs instant help pls

hello everyone,

just to share wid u a probs wid my friend who have justdone her TD shots last day and the probs is about the supplimental which i mean the form to fill up and needed to sing by the civil surgeon and put it in a brown envelop to be included in the process od AOS,what happens my dear VJ family is the doctor does not accpet the form which my friend bring during her TD shots bocz they insisted to have a full medical as they tell my friend that she need it but my friend have her medical already in saint luke and she is all bringing the result of her medical in saint luke,but she is only given a paper that shows she have her TD shots already, i would like to ask anybody here what to do to my friends situition this time so she can have the supplimental form 1-693 and so it will insert in a brown envelop?does she need to visit again another civil surgeon to help her out to produce a supplimental?both my friend and her husband was in probs this time where to ask for help as she is not a member in here so i aM posting her probs so i can help her probs hoping my fellow family could share any helpful solutions....i am happy to hear from my VJ family in here pls do help...god bless.
