DS260 Unable To Find Previous Employer Contact Information

Hi,

 

I filled out all the information regarding my previous employers. For one of them however, the position I was working at was temporary and I don't have any phone number to put for this employer. I'm unable to save this section of the form without it so I just put 000-000-0000 in the telephone number section for the time being. For this same employer, I also don't remember my supervisor's name, because as mentioned earlier, this was only a temporary position for 5 days total. They gave an option of "I don't know" for the name, but not for the phone number. I'm unsure how to proceed with this portion and just want to hear what others have done in this situation, and if you had any problems going forward with this.

I have otherwise completed the DS260 form.

Sorry for the late response. I was able to find the phone number but left the name of the supervisor empty as I couldn't remember.
