You mentioned that they used a stamp for the doctor's signature. Wouldnt the doctor have to sign his name to the form. When I went I had to leave the papers with him, becasue he was out of the office till the next day. His assistants told me that he(doctor) HAD to sign the forms as they have to call in the information to Immigration. Told me they are very strict with these rules. I talked to the doctor before we drove there. He told us to be very careful who you get to do it. He said alot of people just do it to get money and are not a Civil Surgeon. So they mailed them to me the next day after the doctor filled out the form and signed it. Well if you got it done for free, and didnt have to have a doctor fill it out or sign his signature for you, just a nurse. All the more power to ya. :yes:

March 9th, 2010, Received GC that has been lost in the mail for 10 months. Still waiting for my son's that is lost as well.

Filed Waiver for my son's 10 year GC and it was approved. He finally received his GC after its been missing for 2 years.

Thanking God this is over for 10 years.

I hear ya girl! When the embassy called sofyan and i for the interview it was set 3 months in advance!

Filed DCF in Jordan from 7-05 to 3-06, Approved for I-R1.

Same with us Dollface - our interview was set 3 months out too. We thought that would be forever, but it went pretty fast. The hardest thing for us was Abdel getting here just before my final exams. I had to make some sacrifices to have time for him, but it was worth it.

Pink roses, I just looked at the form. She did sign above the stamp. I guess I'll call and make sure that I don't need a civil surgeon to sign off on it too. If I do, that is such a scam! She did everything that needed to be done. We actually have the DS3025 from the medical exam in Morocco, but the doctor there hadn't given him some of the immunizations so we only went to the health department to see if he could get them. He ended up getting a booster for the Tetanus shot, and a chicken pox shot, since he didn't know if he had had them before. This is really just a supplement to show he is getting the boosters he needs.

From what I've seen, the DS3025 from the medical exam is all we have to send in with the AOS application. Is that correct? I know some of you have been through this already so maybe you can help me know what I need to do. I know we don't want an RFE if we can help it.

Thanks Pink Roses! I just called and found out I do have to go to the Civil Surgeon for signature. I called him but his secretary only works 9 to 1 so he said call back tomorrow. I talked to the doctor's son and he thinks we'll probably be able to see the doctor tomorrow morning.

You would think the lady at the health department would have made it clear that we still had to see the civil surgeon. She just kept saying she does these all the time and not to worry she knows what she's doing. I'm just glad we have one here that can fit us in on such short notice. There's only one in this part of Georgia at all, so at least I can be thankful we have that one. I know I've seen some people in the AOS forum say they had to travel really far for one.

Well thats good that you are going to see the Civil Surgeon.

Plus that form is supposed to be sealed by the doctor. You are not allowd to open it.

We had to go to Georgia to find a doctor too. He will fill the paper out and sign it, and seal the envelope. Its written on the outside, not to be opened. Glad you caught that mistake. :yes:

Did the doctor say he was going to charge you to fill it out?
