It appears that mistakes were made on the I-864.

Was the immigrant listed in Part 3?

Did you provide all the information that was listed in the letter?  All pages of the I-130, listing the immigrant in Part 3, the petitioner's SSN, correct household size, and complete tax return or tax transcript?

Yes we put 1 in part 3 and Total household as 2 in part 5 (me and my SO). 
 

Note: the petitioner has also petitioned her mother who’s case is at NVC dq’d. Not sure if we have to count her in the household as well?

His mother is not counted on the I-864 for you since she is not an LPR.  

Review your I-864 for mistakes.  Focus on what was listed in the letter when responding to the RFE.

The RFE clearly lists the issues; go over your I-864 (assuming you kept a copy, as you should) and check all the issues listed.

 

- Did you include all the immigrant's information in part 2?

- Did you check "yes" in part 3, question 1?

- Did you include the petitioner/sponsor's SSN in part 4, question 10?

- Did you enter a 1 in part 3, question 29, and in part 5, question 1, leading to a total household size of 2?

- Did you include all the evidence they listed?
