ds160 alias/other names Question

if you have a different name listed in facebook then what is on your passport do you need to submit that in the other names category.  i.e. John Dover Green but on facebook it's Charlie Green.

 

second question is  where do I put my middle name on the ds 160.  it asks for surname and given name so do I just put surname Green and given name John or will I need to put surname Dover Green and given name John.

If you actually are referred to as "Charlie Green" in normal life, then yes.  If it is just a screenname in social media, then no.

 

Surname is just the last name.

 

EDIT:  I believe there is a section for Social Media handles.  Enter your screenname there.

Put your FB name in the "Social Media" section of the DS-160.  Only put other names used in official documents in the "Other Names" field.

I assume you have a Philippine passport which lists your middle name separately from your given name.  If so, on your DS-160, put your middle name in the field for "Given Names".  Example --

 

Surnames (As in Passport):  Green

Given Names (As in Passport):  John Dover
