Typically there household plus the intending immigrant(s)

13b is applicable.  I would eliminate the N/A and provide the explanation. 

 

I would not fret too much about this.   Your US passport is sufficient proof of citizenship

My options are

11a) U.S. citizen through naturalization. My Certificate of Naturalization number

11b) on picture 

11c) i derived my U.S. citizenship by another method provide an explain

11d) I am a lawful permanent resident of the United States

11e) I am a lawfully admitted nonimmigrant

My bad, fat finger error.   I meant 11b is applicable.  Just delete the N/A.
