Refused to ready to refused

Anyone who got their visa recently what was the pattern. Mine went to ready from refused and then went again to refused from ready. I think it should have gone to issues. Does this happened to anyone else too. 

Did they gve you 221g letter?why your case refused?mine also refused

My interview was on 30 march 2021 I gt 221g my status is still showing refused its been 2 weeks that u courier them doc..did they kept your passport?
