We actually did separate love stories (followed youtubers of course). We did that although it goes against the common consensus that it's not necessary. Our application got approved very recently.

Use this one. It has to be separate, one filled out, dated and signed by you (petitioner) and another one filled out, dated, and signed by her (beneficiary).

What people misunderstand is that USCIS just needs to know how you first met.  This is strictly for them to verify that you did not use a marriage broker agency or website.  If you did use one, then you need to provide evidence that the marriage broker used certain guidelines/regulations.

 

They do not need or want to know the story of how, from that first contact, that you two progressed to the point of a proposal.  Save that for FB/Blog/Autobiography...
