What form do I need from biological father for child's I-130

Dear all,

 

my husband and I would like to move back to the US with my children from a previous marriage. I wrote to the US consulate about which forms we would need to show. They mentioned a notarised agreement by the biological father or alternatively sole custody. I do not necessarily want to ask for sole custody as I think that would be unfair. However, what kind of form would I need to provide their biological dad to sign? Does anyone have a sample version for me?

 

I am kind of lost on this one.

A consent letter Will suffice.  The Dad authorizing his kids to move to the US with u and get it notarized. 

That's what i submitted and it was approved
